my biggest mistake was probably after blowing up account 9, immediately taking out more money and blowing it within a month because i was not being disciplined as it seemed not much compared to what i had just lost.

Exactly! This is a good one. All of my biggest losses that devastated my portfolio came from a blind belief in the trade - trading what i thought was gonna happen. Blame goes on the lack of experience, overestimation of you abilities, having no fear, and ofcourse... greed!
I also like comment from Guerilla Trading. This is exactly situation which I have experienced many times. During time I have learnt to keep my trading breaks and rethink trading plan before I actually open position
