My personal experience with AT&T 15+ years ago: A simple monthly home phone + internet bill had the curious phenomenon of gradually increasing by $0.xx to $1.xx a month even when there was no change of service ever made or requested. If it's repeated across all customer accounts, I think AT&T could have collected an extra 80 million to 100 million per month without providing any extra service whatsoever.
