For NASDAQ listed equities, there's a website where you enter the details on trades you believe are clearly erroneous. However, if you don't succeed in claiming your trade (ruling goes against you) you're charged $250. I'm not sure how it works for futures exchanges... At least on US stocks once you enter a claim for clearly erroneous, it goes to ruling at FINRA (arbitrage). Once there's a ruling, that ruling is final.
