many of these errors or whatever you want to call them, never lead to full P&L responsibility for these guys, make it so that they have to eat it, and risk models/parameters will improve overnight.
lol Ive seen a few 400 mil cable errors in my day... if one had been my fault I wouldnt have lived to tell about it. Somethings rotten in denmark tonight, and its someones head in a dumpster lol.
