    The massively popular ketogenic (keto) diet — often hailed as the fastest way to get lean — increasingly appears to be more of a fad diet and less of a positive, lifelong way of eating. Recently, U.S. News and World Report and their panel of health experts ranked the diet next to last in its annual ranking of overall best diets. As scientists gain a better understanding of how going keto impacts the body and mind, it appears that keto can induce rapid weight-loss — and potentially poor long-term health.

    A new study further indicates that the keto diet may threaten health over time. The study, published in the journal Nature Metabolism and conducted in mice, shows that after going keto, some improvements can happen. After going on keto for a week, the mice's metabolism improved and inflammation decreased.

    However, after a week on the diet, the positive benefits reversed. After two to three months on keto, the mice became obese, with impaired metabolism and skyrocketed inflammation.

    This may read as bad news for keto enthusiasts out there. But the discovery that keto diet is a useful short term intervention, not long term practice, can be seen in a positive light, according to lead author Vishwa Deep Dixit, a researcher at the Yale School of Medicine. As he puts it: "Who wants to be on a diet forever?"

    A FALSE START
    On a keto diet, staple foods like bread and potatoes are replaced with "healthy fats" like nuts, avocado, and butter. The dietary shift tricks the body into breaking down fat and use compounds called ketone bodies as an energy source, instead of carbohydrates and sugar.

    In this study, mice were put on a mouse version of the keto diet, which involved eating a type of chow broken down as 89.5 percent fat, 10.4 percent protein, and 0.1 percent carbs. For about two to three months (depending on the mouse) the team monitored the animal's metabolism and immune system and checked to see if any of their genes had experienced a change.

    Initial results were positive: One week on the diet lead to an increase of important immune cells, called gamma delta T-cells, which are stored in body tissue. These immune cells help reduce inflammation, improve metabolism, and decrease glucose levels, which can help prevent diabetes.

    However, maintaining the diet longer than a week actually depleted these cells, making mice’s glucose levels, inflammation and weight go through the roof.

    Interestingly, the researchers didn't see any weight changes during the first week of keto eating.

    But, after that first week on the keto diet, the mouse bodies got overloaded with fat. That's because they were unable to burn as much fat as they consumed. It seems that turning on the “ketogenic state” activated starvation mode — the mice could rely on ketone bodies for energy, but they also frantically stored fat.

    In turn, the animal's fat stores grew rapidly, eventually causing the mice to be obese, and in some cases, develop diabetes — the exact conditions that the diet initially helped out with.

    WHAT DOES THIS ACTUALLY MEAN FOR HUMANS?
    For now, it's difficult to say. The timeline of when the keto diet might transform from a helpful to harmful metabolic tool isn’t exactly clear.

    But Dixit says that, before doctors can start recommending the keto diet to patients, there needs to be large controlled clinical trials that evaluate whether or not it can cause harm to individuals who are overweight or pre-diabetic. Ideally, further research will indicate exactly what prompts any metabolic or immunological benefits, or harms.

    From the mice study, it seems like the keto diet might be a helpful tool if someone is looking for these changes to happen fast. But if these positive benefits reverse in humans the way they did in mice, the risks of going keto may outweigh this short-lived boost.

    And ultimately, cutting carbs completely can be challenging for people. Other diets — that are more sustainable and less restrictive — may help people achieve the goals they’re after.

    Abstract: Ketone bodies are essential alternative fuels that allow humans to survive periods of glucose scarcity induced by starvation and prolonged exercise. A widely used ketogenic diet (KD), which is extremely high in fat with very low carbohydrates, drives the host into using β-hydroxybutyrate for the production of ATP and lowers NLRP3-mediated inflammation. However, the extremely high fat composition of KD raises the question of how ketogenesis affects adipose tissue to control inflammation and energy homeostasis. Here, by using single-cell RNA sequencing of adipose-tissue-resident immune cells, we show that KD expands metabolically protective γδ T cells that restrain inflammation. Notably, long-term ad libitum KD feeding in mice causes obesity, impairs metabolic health and depletes the adipose-resident γδ T cells. In addition, mice lacking γδ T cells have impaired glucose homeostasis. Our results suggest that γδ T cells are mediators of protective immunometabolic responses that link fatty acid–driven fuel use to reduced adipose tissue inflammation.

  3. Good point. I wonder if there was a control group not on on a keto diet that also ate to its heart's content for comparative purposes. Regardless, however, the high fat consumption evidently did not sufficiently sate the KD mice to prevent them form overeating. And it's a bit striking that the KD mice started to go downhill after the first week.

    But I am not a (mouse) scientist, so I am not in a position to do more than quote the study's findings. Perhaps if they really did get the research wrong, other experts will correct them. Stay tuned!

  4. I find two problems with keto studies:

    1. They look at dirty keto where people eat as much bacon, butter, ribeye, coconut oil and other fats in huge quantities.

    2. They refer to open ended eating of any keto based foods.

    Everything becomes unhealthy unmoderated and shoving unlimited food into these poor mice should produce those kind of results as it would with humans I would expect.

    The quality and quantity combo of calories still matter and to honest, Keto, Paleo, Whole30 or whatever still address the QUALITY side of it and quantity cannot be ignored. 4000 calories a day of saturated fat via dirty keto I would think would do real damage over time.

    The groups Price studied included remote villages in Switzerland, Gaelic communities in the Outer Hebrides, indigenous peoples of North and South America, Melanesian and Polynesian South Sea Islanders, African tribes, Australian Aborigines and New Zealand Maori. Wherever he went, Dr. Price found that beautiful straight teeth, freedom from decay, good physiques, resistance to disease and fine characters were typical of native groups on their traditional diets, rich in essential nutrients.

    When Dr. Price analyzed the foods used by isolated peoples he found that, in comparison to the American diet of his day, they provided at least four times the water-soluble vitamins, calcium and other minerals, and at least TEN times the fat-soluble vitamins, from animal foods such as butter, fish eggs, shellfish, organ meats, eggs and animal fats—the very cholesterol-rich foods now shunned by the American public as unhealthful. These healthy traditional peoples knew instinctively what scientists of Dr. Price’s day had recently discovered—that these fat-soluble vitamins, vitamins A and D, were vital to health because they acted as catalysts to mineral absorption and protein utilization. Without them, we cannot absorb minerals, no matter how abundant they may be in our food. Dr. Price discovered an additional fat-soluble nutrient, which he labeled Activator X, that is present in fish livers and shellfish, and organ meats and butter from cows eating rapidly growing green grass in the spring and fall. All indigenous groups had a source of Activator X, now thought to be vitamin K2, in their diets.

  7. So it's time for an update. I have stayed with the intermittent fasting since starting it a couple years ago and it's become very comfortable. Occasionally I'll do a 24 hour fast, but it's too rare of an event for me to say what if anything it accomplishes. I just do it more for mental fortitude, similar to cold showers which I still do. Actually it's a warm shower with a cold rinse.
    Far as my weight I did get down to 201 several months ago. Looked good but was feeling weak. I've been in the 205-210 range for a year now and that seems to be the sweet spot. Still, at 70 I look in the mirror and say WTF, still an old man body. Not nearly as bad as many I see in the gym, but tight, really tight, just ain't happening. The grind continues.

  9. I've only recently "discovered" tofu and incorporated it into my regular diet. I really like the stuff. And although I add salsa, vegetables and other ingredients to enhance the give it flavor, I kind of like the taste right out of the package. It's bland, to be sure, but in a good way; I can relate. :D
