One of the more uncomfortable, deliberate and distasteful ways to go would be to eat at McDonald's three or more times a day, seven days a week, 52 weeks a year (if that long). By the time that the morbid protocol takes its toll, the subject won't want to live anymore.

Lesson - never speak to psychiatrists in countries where doing so can get you carted off to the funny farm.
