When I am about to place a bid, how can I see something more than smart or one exchange-listed to execute the trade on IB? Long ago, this was sort of an automatic drop-down menu, now I just have one choice or Smart.

Yipes! You're messin' with your Edit/Global_Configuration/Orders and /Presets pages. Save your settings in a spare file right now (or twice! in two separate filenames), and then go screw with them.
Drop-downs here, here, and here: On that last image, I also circled the "sweep to fill" which will put large orders out to multiple exchanges.
Two options, 1) when you add the stock to your page (classic view) if it is available on more than one exchange you can choose which exchange and it will show for example "ABC LSE", with "LSE" being the London Exchange, and "ABC" being the ticker. Doing this you can add "ABC" 2, 3, 4+ times with as many exchanges as it is traded on. 2) once you click "buy" to initiate a bid, you can select the exchange on the drop down menu in the "Destination" column. If you don't have that column you can add it. Edit .. .and 3), 4), and 5) from the post above mine.
