For a moment, I thought your tragedy sounded familiar, and I quickly searched through my medical article archive looking for something I had read months ago. It is my sincere hope that the information will be helpful to your investigation. After reading the article, it is terrifying to think that one of the best hospitals in the US, if not the world, can make such a simple mistake.
