Recommendation request for low-volume, low-cost ES day trading

Please recommend a broker for low-volume day trading ES, that would best fit the following wish list:
- low cost (commissions, margins, ...)
- no account maintenance fees(inactivity)
- demo account (free)
- good reviews, experience with it
- trading from laptop
