How does the law judge people who do something wrong when under total delusion?

He who is without delusion cast the first stone.
Those judged insane are given meds and freed to roam the NYC subway.

It appears to me the first question should always be guilty or not guilty. Then if there are circumstances that should be considered in sentencing you can account for those circumstances at that point.

I was thinking more about judgement day and telling God he can't throw me into the firey pit because of my delusion.
(So it pays for me to have fun now and stay deluded)
Will that get me off the hook?
