the wiki response was a tautology and useless.

Grossly maintained means that upon initial inspection it was maintained
using part of the phrase in defining the phrase is not acceptable.

what is your sense of the meaning?

It sounds like the doctor was examining an area that had been previously diagnosed as having an abnormality of some sort, from an injury or a disease or a birth defect.
